but the guy is real... Valtieri is a vampire and Dark Brother in TES4 and he says that

It's interesting that he seems non-violent, in the sense that he's talking to you. He looks a little wierd too.

yeah, he did look funny... but I guess that's part of the new subtly of vampirism.... you look weird, but you can't be detected so easily.... in Morrowind you could....
